Important to Know...
Documents to Bring to Enroll Your Child

Before you come to enroll your child, you should make an appointment with our Registrar, Vanessa Curtis.  You can contact her at 765-847-2591, Ext 107.  Her office is in the Middle/High School Building.


You need to bring copies of the following:

  • Child's birth certificate
  • Child's social security card
  • Child's up to date shot (immunization) record
  • Proof of residency (such as a current bill with your address)
  • Any guardianship or custody papers, as applicable
If Your Child has a Health Concern...

Here are some things you can do to help us best care for your child...

  • Tell the school nurse and your child's teacher about all health concerns.
  • Introduce yourself to the nurse.  We want to meet you!
  • Bring a copy of any current health care orders from your child's doctor.
  • If your child needs to take medicine at school, please check the page on medication at school.  (Click here to view.)
  • Work with the nurse to develop a health care plan for your child.
  • Tell the nurse, and the teacher, about any changes to the health care plan.
  • Ask questions!
